How To Begin Pediatric In-Home Care
Initial Call & Home
Care Assessment
We start with a simple phone call to learn about your child’s medical needs. Next, we schedule an in-home assessment with one of our licensed nurses. During this visit, we complete a thorough evaluation and create a personalized pediatric home care plan designed for your child’s health and comfort.
Physician Plan of
Treatment Approval
Our nurse prepares a detailed care assessment and a Physician Plan of Treatment (PPOT) for your child’s primary care physician to review. Once your doctor approves, we submit all required documents to the state so your family can begin receiving skilled in-home nursing care without delay.
Caregiver Match &
Ongoing Support
After approval, we carefully match your family with a trained caregiver experienced in caring for medically fragile children. We make sure the fit feels right by introducing caregivers before placement. Every visit is documented for accuracy, and a supervising nurse provides regular check-ins to ensure high-quality pediatric nursing care for your family.
